1.3.6 Children in Our Care (CIOC) Team Case Transfer Procedure

1. Early Planning

  • When the Local Authority care plan proposes a Care Order either with the plan for long term fostering or reunification there should be some consultation with the CIOC Team regarding the final care plan. This should be either:
    1. Invite CIOC Team Manager to the care planning meeting;
    2. Email to the CIOC Team Manager to discuss final care plan prior to filing final evidence;
    3. Add notification of date of final evidence and Final Hearing on transfer list; Email to be sent to Lynsey Sumner to be added to the transfer list and on the CIOC Transfer Panel Meeting Agenda;
    4. Discussion at the Transfer to CIOC Transfer Panel Meeting (meets monthly).
  • Following consultation, joint working should commence from the Final Hearing. The identified CIOC Team SW should attend the Final Hearing alongside the Referral Assessment and Safeguarding Team (RAST) Social Worker (SW);
  • The RAST SW (and Contact Team) will gather information for a life story book / life story work (it is accepted that this work may not be undertaken but the information needs to be collated for transfer).


2. Section 20

  • The RAST SW should liaise with the CIOC Team Manager to discuss the proposed plan for permanence via the CIOC Transfer Panel Meeting. Add notification of date of CIOC Review on transfer list/ email sent to Lynsey Sumner to be added to the transfer list and on the CIOC Transfer Panel Meeting Agenda. Discussion at the Transfer to CIOC Transfer Panel Meeting (meets monthly);
  • The Case should transfer at CIOC Review that endorses the plan for permanence and the CIOC Team SW should attend this Review;
  • The RAST SW (and Contact Team) will gather information for a Life Story Book /Life Story Work (it is accepted that this work may not be undertaken but the information needs to be collated for transfer).


3. Transfer at Final Order

  1. A 4 way meeting to be booked in prior to the Final Hearing for the week immediately after;
  2. Checklist for transfer:
    1. Case notes and summary up to date and finalised;
    2. Chronology up to date;
    3. Care plan to be up to date & Pathway plan part 1 (Age 15 ½ years);
    4. Court Order, statement, assessment and other relevant information should be saved on ‘O’ drive;
    5. Placement plan should include Missing from Home (MFH) procedures;
    6. Directors consent to Out Of Borough placement should be on file;
    7. Statutory visits up to date (pre-warning to be given if visit is due within a week of transfer date);
    8. Personal Education Plan (PEP) up to date (termly) including Pupil Premium;
    9. If next CIOC Review is within a week of transfer, report to have been written. Date of next Review to be provided to CIOC SW;
    10. Joint visit to be completed to child;
    11. Letter to be sent to the birth family by RAST SW informing them of the change of SW and new contact details. The letter will inform them that the new worker will be in touch;
    12. Labelled photos and information relevant to a Life Story book to be given by the RAST SW to the CIOC SW (which will have been gathered throughout involvement);
    13. An up to date record of the child’s journey should be provided e.g. shared moments and/or experiences between the social worker, child and family that will enhance any later life work and enable the child to make sense of their journey. Each SW involved at any stage should write and sign off the history up to the point of transfer.
  3. Transfers to be completed using the Protocol transfer tool and to be accepted immediately after the 4 way meeting assuming all pre-transfer tasks are complete.
